Please have your athlete and members of his coaching staff that will traveling to the United States each fill out this form so that we can request a waiver.

After receiving confirmation of your waiver we will submit this information to the proper organization for consideration. If approved, we will issue a letter to you that can be used for two purposes.

This waiver can be presented to travel from countries under COVID-19 restrictions into the United States at the Airport. Simply print out the letter we issue to your team and take it to the Airport with you.
This waiver can be used to pursue expedited interviews in Jurisdictions where appointments are delayed or totally unavailable. If the athlete still needs a visa, please use this link to set up an appointment https://www.sherrodsportsvisas.com/p1-o1-visa-approval.
